• In cases where the name differs from the Class XII certificate, a gazette notification displaying the name change is required (only PDF format with .pdf extension accepted).

• OBC-NCL certificate

• Caste certificate (for SC)

• Tribe certificate (for ST)

• EWS certificate or PwD certificate (Unique Disability Identification certificate or Forms) issued by a competent authority (only PDF format with .pdf extension accepted).

• Dyslexic candidates must submit FORM-DYSLEXIC 1 and 2 instead of a PwD certificate, with the certificate clearly indicating 'SEVERE' under the Dyslexia category to avail benefits under the PwD category.
